These forms are only if you are unable to complete the New Patient Paperwork online. Please print them and bring them to the appointment if choosing to complete paper forms! Please read over and sign our Notice of Privacy Practices, No Show Policy, and Financial Responibility Policy, acknowledging each of the below policies.
